prints on one page - then correctly fill it out. Applicant must personally sign it - specific instructions
included within form below. Before printing it, you may delete those instructions (it's optional).

It's important to follow instructions. Any omissions or deviations could result in your entry not qualifying. I will review your entry before sending it in - to ensure that it will get accepted in to Green Card the lottery. If there are any mistakes, we can fix them before sending the entry to the U.S.
